Every morning I am wake to my hand being numb and a throbbing pain in my thumb. I believe I have a pinched nerve and have read suggestions on treating it such as heat and stretching the nerve and muscle. But how do I know where exactly to apply heat and to stretch? I do not believe the nerve that is pinched is in my thumb / hand. This has been going on for at least 2 months and is driving me crazy.

Their are two sides of your problem one is simple and the other is serious.
Simple : Weak nerves and blood circulation problem of a mail artery of your arm coming from shoulder, this can be a result of Color bone fracture/injury on the shoulder etc. or numbness due to vayu roga. In all these conditions :
Mustered oil 1 ltr. with 1 kg. of papaya seeds boiled on very slow fire till the seeds are burnt, let the oil cool, strain and massage every day from shoulder to thumb every day 1 hr. before bath, will relieve you. To get good healthy seeds buy big matured papayas from fruit vender’s.
Do fomentation with warm water mixed with rock salt (sendha namak).
The serious problem is caused by candidiasis/ diabetes and thrombosis which ultimately causes gangrene (very serious for those who are suffering but very easy for me to treat)
First get X-Ray and ultrasound done and if it is thrombosis diagnosed for God sake do not go for surgery as all money minded will suggest you surgery (modern science yet has no cure for varicose veins or thrombosis), surgery will be of no help.
